Proof of Identity Requirements
You must provide 2 photocopies (front & back) of your valid driver's license or state ID card when applying for a U.S. passport. The ID provided must be issued more than 6 months ago.
Any of the following will serve as primary proof of identity:
- Previous U.S. passport (mutilated, altered, or damaged passports are not acceptable)
- Naturalization Certificate
- Driver's license older than six months
- Government ID: city, state or federal
- Military ID: military and dependents
Note: If the name on your ID does NOT match the name on your citizenship document or application you must provide a copy of a legal name change. If your state ID has been issued less than 6 months ago, you must fill out the supplemental questionnaire form and provide 5 additional secondary forms of ID.
If you do not have primary proof of identity you can submit 6 - 10 secondary documentation items from the list below. Documents that bear your photograph, signature and/or physical description are particularly helpful.
